Illegible or ambiguous prescriptions are a well-documented cause of dispensing errors. A clear, typed prescription is safer for patients, faster for pharmacists and more professional for your clinic.
This guide is for licensed medical practitioners. A prescription is only valid when issued by a registered prescriber; creating one without authority is illegal in the UAE, India and most countries.
The parts of a prescription
1. Prescriber details (letterhead)
Doctor's name, qualifications, registration or licence number (DHA, DOH, MOHAP, NMC/State council), clinic name, address, phone and timings.
2. Date
3. Patient details
Name, age, sex, and a patient ID or file number. Weight for paediatric prescriptions.
4. Clinical information
Chief complaints, relevant vitals and the diagnosis.
5. The ℞ — medicines
For each medicine:
| Element | Example |
|---|---|
| Name (preferably generic, in CAPITALS) | PARACETAMOL 500 MG TAB |
| Dose | 1 tablet |
| Frequency | 1-0-1 (twice daily) |
| Duration | 5 days |
| Instructions | After food |
6. Investigations and advice
Tests ordered, lifestyle and dietary advice.
7. Follow-up date
8. Signature
Conventions that prevent errors
- Write generic names in capitals to avoid look-alike/sound-alike confusion.
- Never use a trailing zero (write 5 mg, not 5.0 mg) and always use a leading zero (0.5 mg, not .5 mg).
- Spell out "units" rather than "U".
- Avoid ambiguous abbreviations — write "daily" rather than "QD".
- State duration explicitly so the patient knows when to stop.
A free printable prescription pad
The free Prescription Generator remembers your letterhead (on your own device only) and gives you a structured form for patient details, diagnosis and medicines with dose, frequency, duration and instructions. Print it or save as PDF. Patient data is never stored or uploaded.
Local rules
Regulators add their own requirements — controlled-drug prescriptions, e-prescription systems and validity periods vary. In the UAE, many facilities must use authority e-prescription platforms; check DHA, DOH or MOHAP guidance. In India, follow NMC guidelines on generic prescribing.
